There may occasionally be providers who have been added or deleted from the program which are not reflected in this directory so it will be your responsibility to verify whether or not your physician is participating at the time of your service. If your physician is not listed in the directory, you may invite your doctor to participate in the program and have the doctor contact our office for details.
